Have used this plugin for several years and seen it regularly block malicious users. It takes a little effort to setup as per your requirements, but can be very useful if you need to block certain countries, proxies and/or VPNs.

I had my non-profit website’s donnation page an attack by hackers to test stolen credit card. This plugin had denied 99% access to hackers’s access through proxies and VPN.
